acquired Clostridium difficile colitis (CA-CDI): Orville D. Heslop, Karen RoyeGreen, Kathleen Coard, and Michael R. Mulvey Department of Microbiology, University of the West Indies, Jamaica, Department of Pathology, University of the West Indies, Jamaica, National Microbiology Laboratory 1015 Arlington St. Winnipeg, MB R3E 3R2 Abstract Background: Clostridium difficile is the major cause of nosocomial antibiotic-associated diarrhea while secondary Gram-negative shock is rarely seen in cases of community-acquired C. difficile infection (CDI). Toxic megacolon and bowel perforation are also unusual clinical outcomes following community-acquired C. difficile infection. We present a 22-year-old female who acquired CDI in the community and subsequently progressed to toxic megacolon, septic shock and death following admission to hospital. To the best of our knowledge, this was the first case type seen in Jamaica.

A unique strain of community-acquired Clostridium difficile in severe complicated infection and death of a young adult
BACKGROUND Clostridium difficile is the major cause of nosocomial antibiotic-associated diarrhoea with the potential risk of progressing to severe clinical outcomes including death. It is not unusual for Clostridium difficile infection to progress to complications of toxic megacolon, bowel perforation and even Gram-negative sepsis following pathological changes in the intestinal mucosa. These c...

BACKGROUND Clostridium difficile infection (CDI) and Klebsiella pneumoniae carbapenemase producing-Klebsiella pneumoniae (KPC-Kp) bloodstream infection (BSI) are emerging health-care associated (HCA) diseases of public health concern, in terms of morbidity, mortality, and insufficient response to antibiotic therapy. Both agents can be acquired in the hospital but clinical disease can develop in...
متن کاملFulminant Pseudomembranous Colitis Presenting as Sigmoid Stricture and Severe Polyposis with Clinical Response to Intracolonic Vancomycin
Clostridium difficile infection (CDI) is the most common cause of antibiotic-associated diarrhea. Severe diseases carry significant morbidities such as septic shock, acute kidney injury, bowel perforation, and mortality. Immunocompromising conditions increase the risk of developing the disease but whether these individuals suffer a more fulminant course or warrant a more potent first-line treat...
متن کاملIntravenous Tigecycline Facilitates Cure of Severe Clostridium difficile Infection (CDI) After Failure of Standard Therapy: A Case Report and Literature Review of Tigecycline Use in CDI
Standard treatment for severe Clostridium difficile infection (CDI) is oral vancomycin with metronidazole. After failure of this standard regimen, treatment becomes challenging. A young woman treated for septic shock developed CDI. Standard treatment failed and she was ineligible for fecal transplant. Addition of tigecycline to her regimen resulted in cure.
